Today I got my index card back in the mail, signed by Dr. Denton Cooley, the surgeon who performed the first total artificial heart implantation. The address I used was Denton Cooley, Surgical Associates of Texas P.A., MC 3-258, P.O. Box 20345, Houston, TX 77225-0345. The request was sent on November 16, 2011 (10 days total).
